On behalf of all the crew at Collective Care; I am excited to share some excellent news with you; 

As of the 1st of February; Collective Care became: 

What does this mean for you, and is there anything you need to do?

We know that change is scary and that is why we are making this as easy as possible to ease your mind. 

 

There will be no changes to your Support Coordination services from your regular Coordinator or how we support you as a provider. This will be exactly the same!

 

With the recent changes to the NDIS, we felt the name "Pathways" better represents our mission to support you on your journey toward your best life, which is fitting with the goals we want to help you achieve.

 

What is changing:

 

  • E-mails

    • Our e-mails will all end with “@collectivepathways.com.au” if you get an e-mail from this address – yes it is us! 😊If you send one to our old e-mail address, this is okay we can still get these until you get used to the new changes. You will get a reply to let you know of our new e-mail.

    • If you have us saved as a provider, you can simply change these details to our new e-mails.

 

  • New look website!

    • Our new website can be found at www.collectivepathways.com.au

  • New name on invoicing and forms

    • Invoicing details will change with “Collective Pathways” appearing on your plan management portal and statements for any services provided after 1st of February
